The Assistant Care Manager position spans two grades. Assistant Care Manager Mid Sussex Locality (Based in County Hall North, Horsham but will be expected to travel across the Mid Sussex Area)

You will be expected to work alongside social work and health care professionals responsible for delivering a statutory social work service to those people in the Mid Sussex Locality of West Sussex. Mid Sussex Locality covers 2 main areas (Predominately East Grinstead and Haywards Heath and the surrounding parishes) with both rural and urban communities. You will be expected to carry out assessments, care planning, implementing and review care provision within the principles of the care act which relate to the promotion of health, wellbeing and personalisation. The post holder will be responsible for leading on a complex and challenging portfolio of casework, attending multi-disciplinary meetings and working in partnership with our Health, Community and Third Sector colleagues.
The implementation of the Care Act means that we are working within a changing and dynamic environment, with the associated challenges and opportunities that this brings.

For all posts you will act as a representative for the service with a focus on delivering services to those who access and need them. You will also assess, plan, implement and monitor care packages in accordance with statutory requirements.

You will have significant experience of working in a care setting with older people, preferably within Social Care although other relevant backgrounds i.e. Health Care will also be considered.

You should possess a reasonable understanding of the context of Local Authority Social Care, including prioritising need within a context of finite resources. With good IT skills you will also have the ability to communicate effectively to a range of audiences and be able to work in a pressurised but friendly environment.
