We are now looking to recruit an experienced health or social care worker to join our dynamic team, working with older people and local communities.

You will be responsible for coordinating a Home from Hospital service delivered by a team of volunteers to provide short term (up to 6 weeks) personalised support to vulnerable adults with low level needs who are returning home from hospital in the Horsham District.

The service involves first point of contact for older people, assessment and meeting needs, working in our Home from Hospital service which is part of a wider contract with West Sussex County Council for services across the County. We work with partners Guild Care and Age UK West Sussex and the primary aim of the service is to:

Provide services to vulnerable adults that, following a hospital discharge aid their recovery and avoid an unplanned re-admission into hospital and which maintain/improve vulnerable adults health and well-being, by providing emotional and practical support in the home and their living environment.

If you have strong inter-personal skills, advice knowledge and experience, understanding of the needs of older people, and are willing to make a difference locally, we want to hear from you.
