County Assessor with Urgent Response Carer Support – West Sussex 35 hrs per week working from home and reporting into our head office in Climping. Salary: 9.41 per hour. Enhanced rates are available if called out during your on-call week. OVERALL PURPOSE OF POST The principle requirement for the post holder is to assess the needs of the Carer and the person with care needs. Constructing appropriate care plans to support our various respite services including emergency services assessments. This post holder will also be part of the Urgent Response Team and will be an active member within Crossroads Care Duty on Call Team. This involves receiving calls and responding appropriately to the Care Coordinator request to support the clients and staff of Crossroads Care South Central and our statutory partners. This may result in a waking night or day time package of support. On a rota basis, currently one in 4weeks. Playing a key role in the quality assurance, health and safety and professional development training within the organisation, assisting the Lead Assessor in ensuring that the required quality targets are continually achieved. During the duty on-call week, when assessments are minimal, you may be asked to represent the organisation at events around the County and will be expected to present the charity and the services we provide to other professionals and member of the public. A full UK driving licence and access to a car are essential for this position.
