You will be an Assistant Care Manager, your role will be as the duty worker in an integrated Health and social care team with the Northern Community Learning Disability Team.

The role will involve being the single established Duty Worker, co working the duty desk with colleagues from the team who support the duty worker on a rotational basis. Each day there are two duty workers and one back up worker. There is a duty manager (Senior practitioner, Team manager) supporting this role. Duty is the first point of contact for the CLDT, it covers incoming calls, emails and post in to the Community Learning Disability Team and the Learning Disability Transition Team.

You will work creatively with customers and carers to produce person centred solutions.

You will proactively work with other Health and social care professionals in the team (Occupational therapists, Speech and Language Therapists, Psychologist, Community nurses and a physiotherapist).

Partnership working with customers, families, carers, voluntary and statutory agencies.

You will have very good communication skills and an ability to work in a proactive and organised way.
